Financial Aid Forms

Getting your financial aid processed starts with making sure your documents are submitted correctly. This page lists the forms available through the Financial Aid Office and provides guidance on how to submit them. Taking a few extra minutes to make sure everything is complete, accurate, and submitted correctly can save you a lot of time in the long run.

How to Submit Documents

Financial Aid documents are only accepted through the following three methods:

Please do not email forms to the Financial Aid Office. Emailed documents cannot be processed. Submitting the wrong form, an incomplete form, or a form for the wrong award year will delay your financial aid — so always double-check before you submit.

Not sure which forms you need? Check your SWC email or log in to MySWC and visit My Financial Aid to see a list of any documents the Financial Aid Office has requested from you. Make sure you are downloading the correct verification worksheet for your dependency status (dependent or independent) before submitting.

Tips for Submitting Documents

All verification documents must be submitted together as a complete packet — including your verification worksheet, tax transcripts, W-2s, and any other requested items. If anything is missing, your entire packet will be returned to you and must be resubmitted. Incomplete packets are not processed. Once your complete packet has been reviewed and no additional forms are needed, you will receive an email at your SWC email address with your financial aid results.
Take your time when completing forms. Missing information, incorrect answers, or illegible writing will slow down your application. If you are unsure how to answer a question, contact the Financial Aid Office before submitting.
If the Financial Aid Office requests a birth certificate and yours is from another country, do not submit the foreign birth certificate. Instead, bring your original U.S. citizenship document, such as a U.S. Passport, Certificate of Citizenship, or Certificate of Naturalization, to verify your citizenship status.
- U.S. high school diploma: Submit a copy that clearly shows your name and graduation date.
- Foreign high school diploma: Submit a copy along with all supporting documents, including a list of courses, grades, official seals, and signatures. Please allow 2 to 4 weeks for foreign diploma evaluations — plan ahead so this does not delay your financial aid.
